INSURANCE & PAYMENTS

Tuscaloosa Integrative Family Medicine is dedicated to providing you with the highest quality, cost effective medical care. We accept all major credit cards and offer payment installments.

We Accept most *Insurance Plans:

  • Blue Cross and Blue Shield of Alabama​

  • Medicare

  • United Healthcare

  • Humana

  • Tricare

  • Most Commercial Insurance

  • Private Pay

*Please check with your Insurance about your Policy*

Prescription Refill Fee:

There is a $20.00 fee for Prescriptions requested outside your follow-up visits and a $30.00 fee for all other Prescriptions. 

**Scheduled Drugs: We do not routinely prescribe or refill controlled substances or chronic pain medications.**

No-Show Fee:

There is a $35 dollar charge when patient does not show for their appointment. Please click here to see the policy.

Test Results Policy:

The Physician will discuss lab results and test results with you personally at your follow up visits.  If there is an urgent test result, our office will notify your sooner.  You are welcome to review your results via our Patient Portal or request a copy.

Administration Fees:

  • Sports Physicals: $50

  • Disability Forms: $50

  • FMLA Forms: $50

  • PEEHIP Wellness Forms: $25*

    • *Brought in after appointment

  • Medical Patient Letters: $35

  • Returned Check Fee: $35.00 plus cost of returned check.

  • Insurance Claim Re-File Fee: $25.00*

    • *Claim is denied after filing to wrong insurance. Patient did not update insurance.
