Prescription Refill Fee:
There is a $20.00 fee for Prescriptions requested outside your follow-up visits and a $30.00 fee for all other Prescriptions.
**Scheduled Drugs: We do not routinely prescribe or refill controlled substances or chronic pain medications.**
There is a $35 dollar charge when patient does not show for their appointment. Please click here to see the policy.
Test Results Policy:
The Physician will discuss lab results and test results with you personally at your follow up visits. If there is an urgent test result, our office will notify your sooner. You are welcome to review your results via our Patient Portal or request a copy.
Sports Physical Forms: $50
Disability Forms: $50
FMLA Forms: $50
PEEHIP Wellness Forms: $25
Medical Patient Letters: $35
Returned Check Fee: $35.00 plus cost of returned check.
